  • New Client forms

    For new grooming clients, my boss only gets their name, pets name and age, phone number, and breed of pet. I want to make new client forms for everyone to fill out. I am going to have name, address, phone #'s, e-mail (optional), Pets name, date of birth, breed, color, and health problems. Can you guys think of anything else that I should include? Thanks in advance.

    Grooming history and a place for a photo. To really get the folks to fill it out, leave a place to fill in "quirks". Perhaps add a space or options for home grooming, and how the pet reacts (especially to things like nail clipping).

      Current on vaccinations, vets name and #, emergency contact..I print mine on a 4X6 file cards on one side all of the info on the reverse side a emergency release for vet care. It works great

        I like the ones I use. Owner's info including name, address,home, work and cell #. Also dogs age, breed, medical problems. The bottom is for me to fill out example check easy, shy, aggressive etc...

          Veterinarian of choice. It's also good to have a release for emergency veterinary care.

          Size or weight.

          Sex and neutered/spayed.

          Dog or cat aggressive?

            A waiver of some type to sign at the bottom may be a good idea too. I have one at the bottom of my client sheet but I am thinking of having a "check in" sheet w/a waiver at the top and the customers have to sign in at every grooming.. ...any thoughts?

                I like the thought of waivers, but the legality of it standing up has stopped me.
