Forms for drop off

Does anyone use a paper form when clients are dropping off their dogs detailing dogs name, length, face, tail, feet and so on? Does anyone have any examples? I'd like to get some printed on a pad and was thinking of check boxes for everything and printing a ruler on the side of it. I want to make a pretty in-depth one, I don't want any more miss-communication between clients and staff. It's driving me batty!
