I was wondering if anyone could give me a script or questionaire that you give to people calling for a groom?

I'm a small, part-time, by appt. only groomer that only grooms small dogs, so I'm not sure how much this will help you.

I always ask the same stuff:

-dog's name, weight, age.
-Any injuries or illness
-neutered/spayed
-up-to-date on shots
-last time groomed
-groomer ever mention any trouble
-ever known to bite.

I quote price based on their answers.
